HOW TO SIGN UP

We would welcome more supporters and other people connected with the Football Club signing up to participate in the Draw.

For £5.00 per month, you will be allocated a number and have the opportunity to compete in our monthly draw.

We have attached a form below for you to provide some setting up guidance, especially regarding our PAYEE DETAILS, however it would be helpful if you could set up the standing order with your bank or building society first (rather than sending in the form to the Club) and then after doing so, please email our Financial Director Neil Smith at accounts@stourbridgefc.com with a message to advise that the standing order for the 200 Club is in place and the date and the amount of the first payment.

If you want more than one number, just indicate the amount of money you will be paying, and we will allocate your numbers accordingly.

Neil can then get back in touch with you to request any further information required and allocate your 200 Club ball/balls number(s).

This would be appreciated as we have experienced problems with one bank in particular when trying to process new application forms recently from our end.
